Wondrous item, Rare When this pillow is placed beneath the head of a sleeping creature, said creature will experience terrible nightmares. Whoever places the pillow may program one of the following effects to ail the victim.
Disadvantage on ability checks for 8 hr Frightened of one creature you choose -1 initiative for 8 hr Wake up with 2 levels of exhaustion
The pillow may also be used as a weapon. It deals 1d4-3 damage (lowest possible damage being a 1) when a target is hit by the pillow, it must make a DC 13 wisdom saving throw or suffer the effects of the Phantasmal Killer spell.
Created by the trickster god Mephistopheles, the Pillow of Nightmares was deposited into the mortal realm to spread chaos. The pillow had been used to influence high profile individuals through fear, sabotage enemies, and even slay powerful foes. As more chaos was brought into the world, Mephistopheles rejoiced, sometimes even granting his favor to those who excelled in causing misery.

One of my favorite things to do is to make items that have a lot of utility both in and out of combat. The Pillow of Nightmares will allow stealthy characters to manage the dream space of their targets. Also smacking a baddie with a pillow is hilarious.
